If possible, please complete the appropriate forms prior to your dental or orthodontic visit at any Elite Pediatric Dentistry location.  All forms can be completed in our health portal after you schedule an appointment.  Book online, it’s quick and easy!

We ask that new patients arrive 20 minutes early to their first appointment so we can go over your information and verify insurance. We’re excited to meet you!
